Requesting Records by Mail or Online
Requesting Birth or Death Records by Mail
The normal response time for records by mail is 7 days but may take up to 30 days. Access the print application in English (PDF) or Spanish (PDF). Peoria County birth certificates are available from 1982 to present and Peoria County death certificates are available from 2008 to present. To obtain records prior to these years, contact the Peoria County Clerk’s Office.
To complete your request of records by mail, collect the following documents and fee to mail with your request:
- Completed application signed by the applicant
- Copy of a government issued photo ID, such as a driver’s license, state photo ID, or passport.
- Check or money order made payable to "Peoria City/County Health Department." (Do not send cash)

Requesting Birth or Death Records by Email
For a birth record, email vitalhealth@peoriacounty.org. We will inform you how to make the payment. Include in the email:
- The person’s name
- Date of birth
- Mother’s Maiden Name
- Dad’s name if it is on the certificate
- The address where you want it mailed to
- A phone number so we can call you back to get payment information
- And any other information you want to attach (don’t include it in the body of the email) a copy of your valid photo ID, such as a driver’s license, state photo ID, or passport.
For a death record, email vitalhealth@peoriacounty.org. We will inform you how to make the payment. Include in the email:
- The person’s name
- Date of Death
- Your name and relationship to the person.
- The address where you want it mailed to
- A phone number so we can call you back to get payment information
- And any other information you want to attach (don’t include it in the body of the email) a copy of your valid photo ID, such as a driver’s license, state photo ID, or passport.
Requesting Birth or Death Records Online
You can request vital records through an online private agency for certified vital records from most states. This is an authorized source for government-issued vital records with a transaction fee charged.
