Supplemental food packages may include some following foods (listed by category):

- Some Specialized Formulas - This Medically Prescribed Formula form will need to be completed by your doctor and faxed to WIC at 309-679-6695.

Follow Safety Guidelines for Preparing Feeding Formula
Follow the instructions on the formula packaging. Ask your WIC nutritionist or physician, if you have any questions about preparing formula.
